All Registration Paperwork is available online. Please fill out all forms by August 18th, 2024. For those who prefer In-person registration, our "Registration Night" is scheduled for July 30th, 2024 from 4:00 pm - 7:00 pm. Individual appointments to conduct school business are also available upon request after August 11th, please contact the office at 319-827-1314 or email: sec@saintaschool.com
For New Families inquiring about St. Athanasius: please fill out the New Family Information Form. We will contact you as soon as we are able.
To Register a NEW Student:
This includes new preschool/preKindergarten students as well as any student who has not attended a Catholic School before this year. Please fill out the following form to begin the enrollment process. One form per NEW child needs to be filled out. If your child HAS attended a Catholic School within the Archdiocese of Dubuque before this year, you may skip this step.
Once we accept the enrollment, you should receive an email confirmation. You may then proceed with the Registration Process below.
Registration Process for returning students and new students after their enrollment has been accepted.
1. Register each student in Power School.
2. Review St. Athanasius School Policies / Guidelines.
3. Review the classroom supply lists.
4. Required Health Forms (email or send in physical copy to the office by the first day of school):

3-yr old preschool - updated physical form (less than 12 months old); updated immunization form (or religious exemption)

4-yr old preKindergarten - updated physical form (less than 12 months old); updated immunization for (or religious exemption)

Kindergarten - updated physical form (less than 12 months old); updated immunization form with Kindergarten immunizations (or religious exemption); dental screening (once after age 3 - forms available in the office); lead test (once prior to age 6 - form submitted by the physician's office to the state); and vision screening by a medical professional (within 1 year - forms available in the office).

3rd grade - vision screening by either medical professional or school (Lions Club)

7th grade - updated immunization form with 7th grade immunizations (or religious exemption)

7th and 8th grade students participating in school-sponsored activities thru Jesup Community or Bosco Catholic Schools are required to have an updated sports physical form on file.
We can accept your personal doctor's physical or sports physical form or you may use this form.

5. Request Bus Transportation through Jesup Community Schools by printing out the linked form and returning to either the St. A school office or Jesup Community Schools Elementary office.
6. Lunch is no longer subsidized by the federal government. Lunch prices for the 24-25 school year are:

Hot Lunch - $3.00

Seconds- main entree - $1.00; other items - $0.50

A La Carte Items only (including milk for cold lunch) - $ 0.50

Breakfast - $2.00
Applications for Free or Reduced Lunches may be turned into the office or food service manager at any time.
Before/After School Care:
Care for St. A Students is available BEFORE school from 6:30am - 8am and AFTER school from
3pm -- 5:00pm. Additional care for 3 year old preschool students is available from 11am - 3pm on
Mon, Wed and Fri (then transitioning into regular after-school care). Cost is $3/hr per child. Care is available on a drop-in basis as needed. Registration and health forms must be accepted by the office prior to a child attending the Before /After School Care Program.

A DHS physical is required to be on file for 3 and 4yr old preschool students. A doctor's physical form OR a parental statement of health (found HERE) is acceptable for grades K through 8th is required for every child participating in the Before /After School Care Program.
TUITION:
Statements will be available on July 30th. Applications for additional assistance will also be available at that time.
Payments can be made with cash, check, credit card or ACH withdrawal.
